According to Governor Cuomo's Office, starting today (Jan. 11), in New York State we begin phase 1b for COVID-19 vaccine distribution.  This is for:

  • Teachers and education workers
  • First responders
  • Public safety workers
  • Public transit workers
  • People 75 and older

If you fall into any of the above categories, you may be wondering, what next? Before you can get the vaccine, you have to fill out the New York State COVID-19 Vaccine Form. This form can be completed online at forms.ny.gov/s3/vaccine and you will receive a submission ID, or you can fill out the form at your vaccination site.

Individuals being vaccinated must bring proof of eligibility to the vaccination site. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status.

Just in case you're not certain if you are eligible for the vaccine, you can check at am-i-eligible.covid19vaccine.health.ny.gov.
